We must receive a signed credit application from any new account that is requesting terms other than fully paid. Below are two versions of the Credit Application Forms:

The Fill-In version can be filled out directly from your computer, printed, signed and faxed back to us.

Also, do not forget to include a sales tax exemption certificate if you are purchasing the product for resale or if it is Sales Tax exempt for any other reason. The attached list indicates which states require their own forms, which states we use the multi state form, and which states are tax exempt.
